1. I agree to pay all fees associated with the Sound Beginnings program. These fees include the semester registration fee ($15) and the appropriate materials fees for my child(ren).
2. I agree to pay tuition as outlined in my registration. I may pay for the SEMESTER ($180 individual or $220 family), or I may arrange to make 4 MONTHLY installments ($45 individual or $55 family).
3. I understand that tuition for the full semester is due by the 1st class of the semester, or if I have arranged to make monthly payments, tuition is due by the 1st of each month, September through December (Fall semester), or January through April (Winter semester). I agree to pay a $10.00 late fee if my tuition is not paid by the 7th of each month.
4. I realize that enrollment in Sound Beginnings is for a full semester - which means I am responsible for payment of the full semester's tuition even if I miss lessons, take weeks off, or quit in the middle of the semester. I understand that if I decide to not participate, my registration fee and materials fee will also not be reimbursed.
5. I agree to model appropriate behavior and be active in singing, playing, dancing, and engaging in every way possible with my little ones. This is essential in developing trust between parents/caregivers and the child, and between the child and Mrs. Chele. I will have my phone silent and put away during class, unless taking photos/videos, and I will support and encourage my child(ren) and others around me.
6. I agree to read all communication from Mrs. Chele, review the class calendar, attend class, and play the music at home with my child to maximize the benefits of the program. I understand that the more involved and encouraging I am, the more my child will learn.
7. I will inform Mrs. Chele by text, email or phone if my child is unable to attend class. If I am unable to come, I may send another trusted adult to bring my child to class and participate with them.
8. I understand that the teacher, Mrs. Chele, doesn’t answer the phone right before class or while teaching. A message may still be left via call, text, or email and will be answered when class has ended. 9. I understand that visiting young friends/siblings not registered, including babies, may attend class by paying a drop-in rate of $5 per 30-minute class per person, due at the time of class, and that the caregiver must sign a liability waiver. I will let Mrs. Chele know ahead of time if I am bringing any visitors to class.
10. I understand that my child should not bring toys or other distracting items to class.
11. I understand that Dragonfly Laughter Music Studio is a food-free studio, and that gum, candy, and food are not allowed in class.
12. I agree to keep my child home if they are sick. This includes a excessive sneezing or coughing, sore throat, upset stomach, or within the past 24 hours, fever, vomiting or diarrhea.
13. The teacher and studio owner, Mrs. Chele, works hard to maintain a safe teaching environment. However, the Sound Beginnings program does involve a lot of physical play and movement. By enrolling in the program, I, the parent or legal guardian, accept full financial liability for any incident or injury that could possibly happen to my child, myself, or any guest accompanying us while on the Howell property. I release Chele and Bob Howell and all their heirs, assigns and connections, as well as all class participants, from all liability.
